ov chipkaart
you can buy a reloadable gvb card at any gvb kiosk, it's about 7€ and you can continue to refill it for 5 years.  you can also apply for your own card with your photo (request a form at one of the information desks or online) OR you can apply for a ov chipkaart that has a 40% discount for travel during off peak times and people can travel with you and get the discount (up to 4 people- i think).  it's cost 55€ a year, this is the card that i have.

you can also set it up that it automatically reloads.  there are usually reload machines at most albert heijns or you can find one here: http://www.ov-chipkaart.nl/klantenservice/adresvinder/?service&radius=5

also, if  you have someone visiting and you just want a travel card, you can pay 7€ at a gvb kiosk and get a card valid for 24 hours, this is must much cost effective then buying the 1 hour cards on the tram.  i keep a small pile in my wallet and they aren't activated until you check in on the tram/bus/or metro.  i also have spare reloadable gvb cards that i give them to use too.

If you have lost your photo-issued travel card, if it is a basic card (no discounts on travel) contact gvb: 0900-8011.
if you had a discount travel card (i.e.: it usually costs about 55€ for this card or a little more, contact 0900-202 11 63 or try 0900-0980 OR call gvb and tell them you lost your ov-chipkaart with the travel discount.
